Public Awareness Survey

1. HOW TO ENTER: To enter, click on the [Survey] link and complete the survey on pipeline safety and be automatically entered into the sweepstakes. To enter without completing the survey, hand-print your name, complete address, zip and daytime phone number on a 3" x 5" card in a #10 envelope, affix proper postage and mail to: Pipeline Awareness, 11880 Greenville Ave. Suite 120, Dallas, Texas, 75243. No mechanically reproduced entries accepted. One entry per stamped envelope. Limit one entry per person/per household or email address [in any twelve month period,] regardless of the method of entry. Any use of robotic, automatic, programmed or the like methods of participation will void all such entries. All entries received prior to the July drawing will be included in the random winner selection.

2. ELIGIBILITY: Open to residents of the 50 United States and D.C.,18 years of age and older, except for employees of Texas Excavation Safety System, its affiliates, subsidiaries, member companies and agencies (collectively "Promotion Parties"), and members of their immediate family or persons living in the same household. In addition, winners of a previous prize in the Pipeline Awareness Survey are not eligible. Void where prohibited.

3. DRAWING: A random drawing will be conducted on or about the middle of July for each annual promotion. Odds of winning depend on the number of entries received. Non-winning entries will not be included in any subsequent drawings. Potential winner will be notified telephone, email or mail and must sign (or a parent or legal guardian must sign if a minor) any required Affidavit and Release within 14 days of attempted delivery or prize will be forfeited and an alternate may be selected.

4. PRIZE: One (1) Grand Prize total during entire promotion: $500 gift card to a retailer chosen by Sponsor. Total prize value: $500.00. No substitution or transfer of prize permitted by winner. Sponsor reserves the right to substitute a prize of equal or greater value. Prize will be awarded.

5. GENERAL: All income taxes resulting from acceptance of prize are the responsibility of winner. By entering, entrant accepts and agrees to these Official Rules and the decisions of Sponsor, which shall be final in all matters. By accepting prize, winner agrees to hold Promotion Parties, their affiliates, directors, officers, employees and assigns harmless against any and all claims and liability arising out of use or redemption of prize. Acceptance also constitutes permission to the Promotion Parties to use winner's name, likeness and biographical information for marketing purposes without further compensation or right of approval, unless prohibited by law. Promotion Parties are not responsible for lost or late mail, or for technical, hardware or software malfunctions, lost or unavailable network connections, or failed, incorrect, inaccurate, incomplete, garbled or delayed electronic communications whether caused by the sender or by any of the equipment or programming associated with or utilized in this sweepstakes, or by any human error which may occur in the processing of the entries in this sweepstakes. If, in Sponsor's opinion, there is any suspected evidence of tampering with any portion of the sweepstakes, or if technical difficulties compromise the integrity of the sweepstakes, Sponsor reserves the right to modify or terminate the sweepstakes. In the event a dispute arises as to the identity of a potentially winning online entrant, entries made by internet will be declared made by the name on the online entry form. Sponsor reserves the right to terminate the sweepstakes at the end of the current promotion period without further notice. All federal and state laws apply.
